Comments, Suggestions and Complaints 

We welcome your comments both positive and negative as they let us know when we get it right and where there is room for improvement.  Our Suggestions & Comments form can be found in the Waiting Room and when completed can be posted in the red Suggestions Box on the wall.   Alternatively you can download a copy of this form below and either email to or return to the surgery.  ( Please note this email address is not checked daily and is not for any medical matters)

If however, you are unhappy with any aspect of your care or service, please speak to the Practice Manager or another member of the Management Team as soon as possible as we believe this will give us the best changce of putting right whatever has gone wrong.  However if you wish to make a complaint we operate an In-House Complaints Procedure.  This procedure does not deal with any matters of legal liability or compensation.  Full details of how to complain are found in our leaflet How To Make Comments, Suggestions and Complaints located in the Waiting Room or alternatively you can download a copy from the link below:

In the event that you need help with NHS complaints, the Dorset Advocacy Service can offer free information, support and advocacy if you are not satisfied with any complaint so long as it relates to a service provided or funded by the NHS in our area.  For further information on this service please download a copy of their leaflet from the link below.
