How To Register As a Patient
1) Please check postcode checker to make sure your address falls in our practice boundary.
2) For each patient in the household you will need to complete a New Patient Questionnaire AND a pre-registration form. All patients over the age of 16 should use the Adult version of the New Patient Questionnaire, otherwise the child version for under 16's. Please collect the forms from the surgery.
If you have your own blood pressure machine or have taken it recently, please send in your BP reading if possible, for your records. (there is a space on the New Patient Questionnaire to do this)
Once registered with the practice you will be assigned a named accountable GP but you are still free to book appointments with any GP as we do not run personal lists
It is practice policy that where a child is registered at the practice at least one parent is present at the child’s registration and also registers with the practice.

If you change your name, address or telephone number please let us know by phoning Reception, writing to us or changing your details on SystmOnline.
If move outside the practice area, you will need to find a new GP practice. You can find information on www.nhs.uk or your local library.
